How does silicone scar management work?
During the first two weeks of normal healing, the epidermis (superficial layer of skin) is regenerating. During this time, it is too early for scar intervention. Generally after the first two weeks, the new superficial layer of skin is fragile and prone to water loss. This water loss signals cells called fibroblasts to synthesize and release collagen. This rush of collagen to the scar site is the cause of many undesirable properties associated with scarring.
The ability of silicone products to improve occlusion and hydrate the wound likely has an important role in improving the end scar. It is likely not as simple as that, as there are likely other factors at play as well. Relieving tension across a wound and minimizing trauma to the delicate healing skin in the early period also play a role. Additionally, while it may seem like higher occlusion is better, too much occlusion may be detrimental as well.
How to use silicone scar management products
Current gel sheet products are designed to be worn for up to 24 hours, washed and reused. This approach, however can be inconvenient and lead to hygiene issues. Single use silicone gel products aim to be more hygienic and convenient. As in many things in medicine, there is no perfect answer, and products are continually evolving to meet demand.
Ensure skin is cleansed before using any silicone scar treatment. Silicone gel sheets should be worn at all time (or as much as one can tolerate) and washed in between uses. These strips should last 4-6 weeks. Silicone gel products can also be used. This can be done by applying a thin and even layer of product to the scar and let it dry fully, then cover with clothing, pressure garments, or cosmetics as required.
